Becoming A Client The pre-qualification process for becoming an MCM Maxcare self-insured client goes through these steps:

  • Collect Underwriting Information
    MCM Maxcare will work with you to determine and/or develop the following information:

    • Demographic Profile of Your Employees
      In order to receive an accurate quote for the underwriting of your plan, we must determine the demographic composition of your employee group.

    • Your Risk Tolerance
      We will consult with you to determine your tolerance for risk in your self-insurance plan. The greater your tolerance of risk, the greater the potential savings/profits.

    • Loss Claims History
      We must review the loss claim history of your previous health insurance plan(s).

  • MCM Maxcare's Proposal to the Underwriter
    After collecting the above information, MCM Maxcare submits a proposal to the third party underwriting company.

  • Your Proposal from MCM Maxcare
    Once MCM Maxcare receives the underwriting quote, we will prepare the proposal for your self-insurance plan, which will include:

    • Plan Synopsis
      An overview of your plan, highlighting the unique characteristics of your plan.

    • Schedule of Benefits
      A specific delineation of the benefits for your customized plan with associated co-pays, co-insurance and/or deductibles. Learn more about co-pays, co-insurance and deductibles.

    • Summary Plan Document
      A multi-page, detailed document providing all the services provided by your plan and the procedures members must follow in order to enjoy these services.

    • Provider List
      A complete list of the health care providers and facilities in our network.

    • Stop/Loss Coverage
      Provided by the underwriting company, this coverage is based upon the thresholds determined by your risk tolerance.

  • Your Approval
    When you approve our proposal, we will be pleased to welcome you as a new client of MCM Maxcare.
