How Your Plan Works
How Your Plan Works If you are a new MCM Maxcare member, you should follow these steps to make sure you are properly informed regarding how your plan works:

  • Receive your member ID
  • Receive and review your Summary Plan Description handbook
  • Review your pre-certification rules
  • Review your co-pay schedule
When you need to visit a health care provider or facility, follow these steps:

  • Confirm your doctor or facility is in our network (Search for Physicians)
  • Visit your doctor (remember, in order to visit a specialist, you do not need a referral from your primary care physician, provided that specialist is in the MCM Maxcare network)
  • Present your MCM Maxcare member card and related insurance information for billing purposes
  • Provide your co-payment
After your visit, the following sequence of events occur:

  • Your doctor bills MCM Maxcare for the service provided
  • MCM Maxcare pays your doctor
  • MCM Maxcare mails you an explanation of the benefits you received along with a complete billing record for those services

Important Things To Remember About Bills You Receive From Your Health Care Provider

  • If you did not pay your co-pay when the health care service was received, you must pay your provider's bill for your co-pay
  • If you receive more than 2 billings from your provider for amounts other than your co-pay, contact your provider to check the status
